Abstract

Diabetes mellitus (DM), especially non insulin dependant, is a public health problem and target organ complications, secondary to diabetes, are of the most important medical concern. Of these, diabetic nephropathy (DN) has become the single most frequent cause of end stage renal disease (ESRD) and is now considered a medical catastrophe of worldwide dimensions. Abnormal activation of inflammatory mechanisms is now gaining recognition as a possible unifying mechanism for the development of type 2 DM as well as its attendant renal complications.
The current study was undertaken to assess whether the serum levels of the inflammatory factors tumor necrosis factor-alpha (TNF-α), interlukin-6 (IL-6) and endothelin-1 (ET-1) in relation with metabolic parameters in non insulin dependant diabetic patients are correlated with diabetic kidney disease as reflected by their urinary albumin excretion (UAE).
In order to fulfill our aim in this study, it was conducted on 67 subjects divided into two groups: Group I consisted of 17 healthy volunteers served as control group. Group II consisted of 50 type 2 diabetic patients chosen from diabetics attending the out-patients’ diabetes and endocrinology clinic at Ain Shams University Hospital (ASUH), in the period from February to July 2006. These were further subdivided into three subgroups according to their UAE. The first subgroup was diabetic patients with normoalbuminuria (group II a) which consisted of 20 diabetic patients. The second subgroup (group II b) consisted of 15 diabetic patient with microalbuminuria. Finally, fifteen diabetic patient with macroalbuminuria represented the third subgroup (group II c).
