Abstract

The present: work was carried out. on on adult pal ieuts of class I and I 1 physical status accord i mi i <i flic A s A c lass i f i cat j on , riicir mie,*i ranged t i ”in 211 i o 54 years . They were classified into two main yi mips , ”Morphine group , and Bui orphanol group” . *■>. irh oi”up was c 1 assi tied intd ’» subgroups , id patients each
Morpli i ne group :
Patients included iu morphine subgroups t , * am :i recei ved mo r phi ne su 1 pliatie 1 ntrfivfiiousl v i n doses <d o. \ , 0. 15 , and 0.225 muiu/kgni body weight t esper t ively
But. orphanol group :
Pat ients incl uded i n but or phano 1 suinu oup--. i and 3 rocei ved but orphanol tar ta rate i nt.ravenous I v in doses of 0.02 , 0.03 . and O.D45 moiii/kgm lied. wn<ihi respecI i vely .
The a im of the present work was to assess i lie suppressive effects of morphine sulphate fnnip.it ni with those of but orphanol tar I.a rate, on the response i o
endotracheal intubation with increasing dn.scn m both drugs as regards ; arterial PI ood pressure changes i.sv.sinl ic diast ol i c , and mean art or I al bl ood pressure > , hour t rate changes , tearing , coughi ng , and siroin i no
*Atropi ne su 1 phate 0. 6 liigm . and diazepam in mom . were injected intramuscularly in all patients one hour pr i or to i nduct ion of anaesthesia .
Anal gesia was acni eved by mo r phi ne su I pi mi •■ or butorphano1 tartarate i njected i ntravenous Iy in doses corresponding to the relative subgroups .
Induction of anaesthesia was aciii eved by a sleeping dose of thi open tone sodium (4-7 mgm/kgm body we I glii > .
in tubat i on was carried out after adequate re I OKat i on by sue c i ny l choline in a dose of J mgm/kgm bodv wiunhi fol 1 owed by full oxygenat i on of the pat i ent
The following parameters were recorded ; blood pressure values < systo1ie , diast oli c , and mean arterial) , and heart rate . Recording of these paramoiers was done before i ntubat i on ,’ then 30 , o0 , MO arid 1 210 seconds after tntubat ion
Tearing , coughing , and si raining WIJI e nii.vi vi-il during intubation , and immediately -after .
in all subgroups , the cardiovascular parameter’s showed signi f icanr increase 30 seconds after i nt.ubat i or; with gradual decrease to nearly their initial values 120 seconds after intubation ,
Statisti cal anaiysi s of data obta i nerl showed si gn i f i cant, di fferences aniongest the i nd i v idua 1 rnorph i ne subgroups . Similar findings were observed with but orphan o) .subgroups . However . statistical com pari sun between each morphine subgroup and i ts correspond i rig butorphanol subgroup revealed no si gn i f i cant di f ferenee
Tear i tig :
r*
All patients (100%> included in morphine subgroup ”l” responded to i ntubat ion by the product i on of tears , compared wi th 6 pat ients i n subgroup ”2” . However , noreof the patients in subgroup ”3” responded to i ntubat i on by tear i tig
Pat i ent.s of butorphanol subgroups who showed tearing in response to i ntubat’t on were 4 , A and ft in subgroup l , 2 , and 3 respectively .
Coughing :
Coughing was not recorded as a react: ion to
in tuba r.i on , but there was a de I ayed return of cough ref 1 ex on recovery with ’3 patients of frutorphanoi subgroup ”3” -
Stra ining ,; ***
Straining was not observed a.s u react, ion to intubation in any of.our patient.s .
