Abstract Study limitations A long term prospective study would be required to determine whether subjects with abnormal endothelium-dependent vasodilatation are at increased risk of developing clinically evident atherosclerotic disease in later life. Most investigators have found that endothelial dysfunction is a diffuse disorder that may affect the brachial, femoral, and coronary circulation even in asymptomatic subjects at risk of vascular disease . This is consistent with damaging effects of risk factors such as hypercholesterolemia ,diabetes mellitus and systemic hypertension. However, one report has suggested that endothelial dysfunction at least in I the coronary circulation may occur in patchy distribuation .If this was true in other systemic arteries, it might influence the ability of this technique to detect early vascular dysfunction. Furthermore, in this study we defined the cuttoff point for endothelial dysfunction retrospectively by looking at the best test results,we need next step ,a prosp~ctive study to reevalute this cuttoff point for endothelial dysfunction at FMD%:S 4.5%. 90 |
