Abstract Abstract Between Feb 2004 .to Oct. 2006. Forty patients with locally advanced, stages III and IV non metastatic head and neck squamous cell carcinoma (HNSCC) including; oropharynx, hypopharynx, and larynx , were enrolled in this study to receive Concurrent chemoradiotherapy using Gemcitabine 100 mg per square meter given as intravenous infusion over 30 minutes 2 hours before radiotherapy sitting this is given weekly throughout the radical conventional radiotherapy course of 70 gray over 7 weeks Results: Overall response rate was 70%. Out of the 40 patients CR was achieved in 20 patients (50%) and PR in 8 patients (20%). Seven patients (17.5%) had progressive disease. And 5 patients (12.5%) show stionary course of the disease Complete response was achieved in 72% (16/22) of those patients having tumors of the larynx compared to 19% (3/16) for those having hypopharyngeal cancer; tumor stage was also a significant prognostic factor where 68% of T3 patients showed CR while only 28% of T4 patients achieved CR. acute skin reactions were experienced by all 40 patients (100%). With grade 3 and 4 reported in 15 patients (37.5%) All patients developed some degree of Mucositis with grade 3 and 4 reported in 55% of patients |
