Abstract Aim of the work The carpal scaphoid is the most frequently fractured carpal bone and has the highest rate of non-union of any wrist fracture; this thesis aims at evaluation of operative treatment of un-united scaphoid fractures by bone graft from iliac bone or lower radius in EL-Minia university hospital. Mechanism of injury, classification, clinical presentation and various modalities of investigations and treatment will also be discussed. Conclusion Our results were evaluated following the criteria of Kaulesar Sukul et al., (1990). The overall results were as follows: 30% of patients had an excellent result, 50% a good result, 10% a fair result and 10% a poor result. We used two systems of grafting techniques in our study either liac bone graft or lower radius graft, we found that the results were the same. But we recommended the lower radius graft for the following advantages: 1- The graft taken from the same exposure without the need for another exposure in iliac crest. 2- We can use local I.V. anesthetic and avoid the complications of general anesthesia on the patient. 3- The amount of graft needed not too much, so, graft taken from the lower radius was enough to fill the gap. 4- Short operative time. 5- Avoid the complications of iliac bone graft. We recommend using iliac graft in difficult cases particularly with long duration of non union, old age of the patient in whom the cancellous bone in the lower radius not so good and when there is marked cystic changes. |
