Abstract

The work will present a study on thirty four patients suffering from different orthopaedic problems, treated according to the principles of Hizarov and with the use of his ring fixator. The aim is to evaluate this system (of Hizarov) in different orthopaedic Problems.
From the previous discussion, we can conclude the following:
) The use of the Hizarov fixator in fractures: 1)
The Hizarov fixator is recommended in closed communed fractures of long bones especially para-articular fractures, and in open fractures with or without bone loss.
Simple uncomplicated fractures are better treated by simple, less technically complicated methods, as casts and internal fixations.
The Hizarov fixator is not recommended in intrarticular fractures (as in case 4) because of the difficulties in reduction using the olive wires, and the need for operative elevation of the depressed articular surface. Thus, it must only be used as an alternative method when extensive dissection and internal fixation are contra-indicated.
In Treatment of fractures, although the Hizarov fixator is bulkey. Complex, and less tolerable by the patients than pin fixators, Yet it has the advantages of being; (1) more physiologic, as it allows early weight bearing ; (2) reduction and correction of angulations can be performed during its application ; and, (5) with higher rate of union, and icsser rate of infection.
(2) The use of Hizarov fixator is pseudarthrosis:
The Hizarov methods of treatment of septic and aseptic pseudarthrosis, and pscudarthrosis with bone defects (inblock removal of the necrotic bones, corticotomy, compression- distraction, and bone trans-portation produce much better results than
the conventional methods.
Both of the Hizarov fixator and pin fixators can be used with the Hizarov methods, with almost the same results. However, the Hizarov fixator, again, has the advantages. Of allowing early weight bearing, and gradual correction of deformities during course of treatment.
