Abstract

According to this experimental study, we can determine which type of laser sclera sclerostomy yields significantly longer and lasting filtration with fewer complications. Then we would be able to use this modality to perform laser sclerostomy in glaucomatous patients (clinical study).this laser modality will be applied to perform sclerostomy on blind glaucomatous group ,and then on visual functioning glaucomatous group. Both the success rates and complications of this procedure in both clinical groups will be described and discussed.
Lt is concluded from the study many points which we want to emphasize on them. they include:
Usage of a laser with less thermal effects on the tissues as ER:YAG laser is better than usage of a laser with more thermal characteristics, such as Nd:YAG laser or the diode leser for performing sclerostomy.
Laser sclerostomy ab externo offers the prospect of quick, simple, and minimally invasive procedure in the treatment of glaucoma, but requires precise attention to many details. The ideal selection of the patient, ideal pulse energy, ideal repetition rate of pulses, the ideal pulse duration, proper probe placement and orientation, recognition of completion of the sclerostomy, and close postoperative follow up and management of complications are all critical in performing the procedure well and safely.
The results of virgin conventional filtration surgery are as good or better than those for laser sclerostomy surgery. Until laser sclerostomy techniques exceed the success of conventional surgical technique that have been enhanced by ant metabolites, these procedures may be the second by ant metabolites, these procedures may be the second or third line surgical options.
Extensive laboratory investigations are needed before leser sclerostomy be widespread in clinical use. Additional refinements are being made, and still necessary, in this field.
