Abstract

Aim of the work
Our study is designed to compare the recent stapled technique versus conventional haemorrhoidectomy in patients with 3rd and 4th degree piles as regard the short term outcome including:
1- Duration of Operation.
2- Post operative Pain.
3- Length of hospital stay.
4- Complications, including:
- Urine retention.
- Reactionary hemorrhage.
- Pruritis and wound discharge.
- Pain at defecation.
5- Wound healing.
6- Patient’s satisfaction.
7- Time of return to normal activity.
This was done through interviewing patients one week, three weeks, six weeks and three months after operation.
Conclusion
Conventional haemorrhoidectomy by the ligation and excision technique is considered and effective method in the treatment of 3rd and 4th degree piles but owing to its complications it was shaken off its rank. Stapled haemorrhoidectomy (circumferential mucosectomy) is a novel technique that achieved prompt control of haemorrhoidal symptoms and is superior to the conventional technique regarding post operative complications.
Duration of operation was shorter in the stapled technique than the conventional technique. Also the post operative pain was different in the two groups being severe lasting for long period and agonizing with more analgesia requirement in the conventional group, while it was dull, tolerable and lasting for shorter period and no need for more analgesia in the stapled group. The control of patients symptoms like prolapse, bleeding and anal discharger took place in the two techniques but faster and effective control with minimal post operative complications in the stapled technique. This was reflected on patients satisfaction as most of the stapled group were satisfied, regarding the results of operation that helped them to resume their normal activities early than the conventional group.
