Abstract

This study aimed to discover not only the predominantly expressed T helper cell subtypes among the chronic rhinosinusitis patients but also to find out the correlation between the chemokine receptors expressed by these cells and the pathogenesis of rhinosinusitis. These findings may help in creating new models of treatment for these cases especially that the ordinary treatment models for rhinosinusitis are still unsatisfactory for large scale of patients.
From this research we concluded that:
1- CCR3 and CCR4 chemokine receptors are highly expressed among the tissues of a topic patients and this expression may stand behind the eosinophil recruitment in these tissues which may be the main factor responsible for the pathology of this a topic rhinosinusitis.
2- Severity of the a topic rhinosinusitis is positively correlated to the expression levels of the CCR3 and CCR4 chemokine receptors and this may reflect the important role of these receptors, either direct in the pathogenesis of these cases.
3- TH1 cells are the predominant infiltrating cells in cases of non-atopic rhinosinusitis and this may reflect the straight forward pathogenesis of these cases and may give an impression about the use of some useless drugs in treating these patients like antiallergics.
4- Depending on total IgE level for classifying patients with chronic rhinosinusitis can be considered promising as according to this on this molecular data we can classify the patients according to the underlying pathology or in other words, according to the underlying T-helper cell expression that may help a lot of these cases with chronic rhinosinusitis.
5- The TH2 paradigm in a topic rhinosinusitis not only allows a better understanding of the basic mechanisms that are involved in the pathogenesis of these conditions but also represents a conceptual basis for the development of novel nonallergen-specific or allergen-specific therapeutic strategies.
6- Real time quantitative PCR proved to be powerful tool in profiling the expression of chemokine receptors among T-helper cell subsets even if the differences in the basal levels of gene transcription are extremely small, they can be detected and quantified by real time PCR efficiently. For that reason we can say that real time PCR can be used in diagnosis of patients with chronic rhinosinusitis depending on this molecular bases but the only obstacle will remain is that procedure will be little pit costive.
