Abstract

The aim of the work is:
Clinical, Radiological and neurophysiological assessment of patients with extra pyramidal movement disorders, correlating the clinical severity of movement disorders with radiological and neurophysiological results emphasizing the value of trancranial magnetic stimulation in prediction of outcome.
In conclusions: the present study suggested that TMS may be a sensitive method for the detection of Parkinson’s disease and dystonia, also it may help to differentiate between Parkinson’s disease and other parkinsonian syndromes.
Clinical severity of PD and dystonia and some TMS measurements can give some prediction of the cognitive as well as psychiatric states of those patients.
Silent period abnormalities represent abnormal cortical excitability, which is common for patients with Parkinson’s disease and dystonia.
CMCT and Silent period duration may be sensitive tools to establish the diagnosis, assess clinical severity, cognitive function and psychiatric state of Parkinson’s disease patients. While silent period duration may give a clue to the psychiatric condition of dystonic patients.
We recommend the measurement of CMCT and SP duration in Parkinson’s disease patients especially atypical cases to help establishing the diagnosis and predicting their cognitive and psychological functions.
SP duration may be helpful in prediction of the psychiatric state of dystonic patients.
