Abstract

Premature rupture of membranes with leakage of amniotic fluid before uterine contraction. It is common event occurring approximately in 10% of all pregnancies.
This study included 60 pregnant women from the attendants of EL-Minia university hospital.
They were divided into 2 groups 30 pregnant women between 37-40 weeks gestations with PROM as suspected from history and confirmed by either fern test or ultrasonic examination. The other group included 30 pregnant women between 37-40 weeks gestations with intact membranes.
from every case sample was taken from cervicovaginal discharge in the posterior vaginal fornix by a sterile swabs, emersed in 0.5 ml of phosphate saline buffer for 5 minutes and immediately refrigerated before taking to laboratory fetal fibronectin concentration was detected by single radial immuno diffusion.
We found that 28 out of 30 pregnant women with P ROM have positive test results for fetal fibronectin while 6 out of 30 pregnant women with intact membranes have positive test results for fetal fibronectin.
The mean fetal fibronectin concentration was 6.9 +(-)2.5 mg/dl in pregnant women with PROM compared with those with intact membranes who had mean fetal fibronectin concentration of 1.3 +(-)1.6.
The sensitivity of detecting fibronectin in cases of premature rupture of membranes as a method for diagnosis of these cases was 93.3%, specificity was 80%, positive predictive value was 82.4% and a negative predictive value was 92.3%.
from our study there was a negative significant correlation between fibronectin concentration and gestational age. It was detected that fibronectin concentration is decreased with increasing gestational age.
We conclude on the basis of these findings that fetal fibronectin is a sensitive and specific marker for detection of PROM especially in cases where the diagnosis of doubtful.
