Abstract

It is well established that inflammation plays a major role in coronary atherosclerotic plaque destabilization and in the development of clinical syndromes we recognize as acute coronary syndromes (ACS) (unstable angina (UA) and acute myocardial infarction (AMI)). A complex intravascular inflammatory response is the integral component of the disruption of the fibrous cap, which is the major pathophysiological event of the dynamic instability.
The aim of this work was to study the serum levels of IL10 (anti-inflammatory) and neopterin (pro-inflammatory) and their correlation in patients with acute coronary syndromes.
Subjects and Methods: This work included 70 subjects, 60 patients with acute coronary syndromes (groups II , III, and IV) and 10 healthy volunteers to serve as a control group (group I). serum neopterin and IL 10 levels were measured by ELISA.
Results: The mean values ± SD of serum neopterin (nmol/l) in groups I, II, III and IV were ( 5.6±2.11, 14±5.2, 22.1±6.98 & 21.8±9.1 ) , respectively. The mean values ± SD of serum IL10 (pg/ml) in groups I, II, III and IV were ( 167±46.3, 52.5±34.8, 37.4±13.2 and 46.1±36.9 ), respectively. In the groups II ,III ,and IV, IL-10 levels were significantly lower than that in the controls, and neopterin levels were significantly higher than that in the controls and there was significant negative correlation between IL10 and neopterin.
These data may suggest an imbalance between IL-10 (anti-inflammatory cytokine) and neopterin (pro-inflammatory cytokine) .
Conclusions : Our data showed an increased production of pro-inflammatory mediators in ACS that may be detectable in circulating blood ; this finding was associated with a reduced production of the anti-inflammatory cytokine IL10. In conclusion, a relevant imbalance is present in ACS and this fact could contribute to plaque instability and clinical manifestations.The Complications Of Atherosclerotic Vascular Disease, Such As Coronary Artery Disease,
