Abstract

Regulation of fetal growth is complex and multifactorial . Diverse factors, including intrinsic fetal conditions as well as maternal and environmental factors, lead to aberrant intrauterine growth. the mechanisms controllong intrauterine growth are incompletely understood, but an interaction of maternal . placental and fetal endocrine factors is likely to govern partitioning of nutrients and rate of fetal cell proliferation and maturation.
The main established endocrine regulators of fetal hrowth include insulin and insulin like growth factors system where IGF-I cord level correlate well with birth weight, insulin,s role is thought to be permissive in fetal growth, and the IGF-IIlevels correlate poorly witj birth weight and in many studies it was proved that insulin like growth system is important early in gestation and not in late pregnancies.
Leptin, a 18 KD ahormone secreted by adipocyles and placenta, has recently een positively associated with intrauterine growth. this hormone is detectable in cord blood, however the mechanism by which it may be related to fetal growth is not known . Insulin may increase leptin in humans and leptin is directly associated with hrowth hormone and the with circulating concentration of insulin like growth factors in extrauterine life, because the role of Leptin in determining fetal growth has not yet been evaluated well. we sought to detrmine whether leptin,srole in fetal growth is independent and wether it is predictors of fetal growth in the early neonatal period and its interaction with the other metabolic hormonea.
the role leptin among the complex network of factors controlling fetal growth is incompletely understood. leptin is associated with birth weight, but the mechanisms underlying this association remain unknown.
this association may reflect either a simple relationship with asipose tissue or an active role for leptin in fetal growth.our study was carried out aiming to demonstrate the active role of leptin in conjunction with some oy\ther metabolic hormones in early neonatal life.
it was carried out in ther suez canal universityn hospital , the neonatal intensivecare unit, on 98 newborn; half of them were full terms and the oyher were premature; the paternal consent was recrived before engagment in our study. all the study group was subjected to full history talking full physical examination, anthropometric measurements including weight , lenght and BMI both at day 1 and day 7 of postnatal life. also blood samples were collected twice from all the study group at day 1 of life and at day 7 of life , for detection pf different hormonal levels.
Leptin was measured by EASIA technique; and tge other metabolic hormones including insulin , cortiso, FT4 and ACTH were performed by the automated innunoassay analyzer (IMMULTTE). all data was gathered using a specific computer progra SPSS.
we studied therelationship of leptin with gestational age, with the other metabolic hormones and with the anthropometric as indictiors of physical hrowth in the ealy neonatal period of full terms and premature. we also studied the previous relatioins in special groups as infants of diabetic mothers in comparison to infants of healthy mothers and as well as in the neonates divided into LGA, AGA, and SGA according to relation of birtg to gestational age .
strong corrlation between leptin hormone and gwstational age, its level increasing gastational age , also it is correlated withthe anthropometric masurments of all the study group both in day 1 and say 7 . leptin is positively related to the other metabolic hormones especially with insulun indicating the active role of leptin in growth.
