Abstract

This work was conducted to evaluate a new score combining the Doppler umbilical artery score and the uterine artery score to generate a new placental score using Doppler ultrasound evaluate both to placental score and uteroplacental score.
In this study 40 pregnant women attended to Obstetrics and Gynecology clinic of Suez Canal University Hospital for ante-natal care were included. The age of the patients in this study range from 18-45 years old and there gravidity ranged from gravid 1 to gravid 5.
The gestational age was during 3rd trimester of the pregnancy. The period of the study started at January 2005 and finished at January 2006.
The patients of the study were classified in to four groups of high risk pregnancy as follow :
1- Hypertensive group (10 patients)
2- Diastolic group ( 10 patients )
3- Pregnant women complicated with IUGR (1 patients 0)
4- Other high-risk subgroups
- Vaginal bleeding (3 patients)
- Postmature pregnancy ( 3 patients)
- Oligohydramnios ( 2 patients )
- Absent or decreased total movement (2 patients)
All of the patients were subjected to history tasking, through general & obstetric examinations and investigation. All of them were subjected to ultrasonic examinations and biophysical profile scoring, placental grading degree of maturity & Doppler study.
Doppler study was done for them included Doppler umbilical artery & Doppler uterine arteries. Both scores were combined to to create the new placental score.
This new score is more sensitive and accurate to predict the pregnancy outcome (C.S , SGA or preterm delivery) in high risk pregnancy .
The results were collected and analysis statistically using X2 test and receiver operator characteristic (ROC) to estimate the accuracy and significance of umbilical score alone, uterine score alone and the new placental score , by meaning of the area under the occur .
On this study we can concluded that:
1- New placental score was the most sensitive , specific and significant test, followed by the umbilical artery score and lastly by the uterine artery.
2- All these tests showed more sensitivity to the C.S as a pregnancy outcome in high risk pregnancy followed by SGA and lastly for preterm delivery .
3-This new placental score is of great value to predict the pregnancy out-come, especially when the umbilical artery Doppler is within normal range
