Pathogenesis of the cleft lip nasal deformity is not only due to an extrinsic factor, which is presented in the misdirected pull of the muscle, but also due to an intrinsic deficiency of the alar cartilage, which makes it prone to the deformational forces by the malinserted perinasal and orbicularis oris muscle. This was concluded through:
I. Through the surgical observation; a three dimensional difference of the lower lateral cartilage as regard their
position and size.
II. The anthropometric studies; it was significantly reported that the lower lateral cartilage of the cleft side to be broader, shorter and it tend to be thinner than the alar cartilage of the non cleft side.
III. The histological analysis; it was reported that there is significantly difference (deficiency) of the collagen and glycosaminoglycans content of the lower lateral cartilage of the cleft side.
Fibroblast Growth Factor 18 was used as an indicator for the alar cartilage maturation and proliferation. Although the results were non significant, we considered it a start for further studies to investigate other growth factors which might affect the maturity of the lower lateral cartilage e.g.: bone morphogenic protein (2, 4, 5, 6, 7), parathyroid hormone related protein,
hedgehog family of proteins, etc…
The alar cartilage might not only be deficient in content of collagen but also deficient in the type of collagen. We recommend further studies to quantify the main two types of collagen I and II, and we recommend using fresh frozen specimens and the immune
florescent technique to detect the Ag-Ab binding which is more accurate than the ELISA technique.
