Abstract This study was carried out on sixty patients with chronic liver disease, admitted at Ain Shams University Hospitals. The aim of this work was to study the value of Right liver lobe diameter/ Prothrombin time ratio in comparison to Right liver lobe diameter/Serum Albumin ratio and Manual platelet count/ Splenic bipolar diameter ratio as noninvasive predictors of presence of esophageal varices and their risk of bleeding. The patients were divided into two groups: group 1 included patients with no esophageal varices, while group 2 included patients with non bleeding esophageal varices (Group 2A) and patients with bleeding esophageal varices (Group 2B). All the patients were subjected to full history taking,physical examination,routine laboratory investigations including liver function tests renal function tests, coagulation profile complete blood count and protein/creatinine ratio as well as abdominal ultrasonography and upper gastrointestinal endoscopy. Our results revealed that Right liver lobe diameter/ Prothrombin time ratio, Right liver lobe diameter/ Serum Albumin ratio and Manual platelet count/ Splenic bipolar diameter ratio were able to differentiate between patients with no esophageal varices and patients with non bleeding esophageal varices with sensitivity 90%,80% and 90%respectively, specificity 60 %, 70% and 73.3% respectively and accuracy 70%, 73.33 % and 78.87% respectively. Our results also revealed that Right liver lobe diameter/ Prothrombin time ratio and Manual platelet count/ Splenic bipolar diameter ratio were able to differentiate between patients with no esophageal varices and patients with bleeding esophageal varices with sensitivity 90% and 90% respectively, specificity 80 % and 73.3% respectively and accuracy 83.33% and 78.87% respectively. |
