Abstract

Summary
The long term indwelling urethral catheter continues to be a major cause of morbidity in patients in hospitals. The complications arising from catheterization of the urinary bladder have been extensively researched and reviewed. Although catheterization is a common procedure, a high level of nursing knowledge and skill is required to achieve effective and safe management. This study reviews the principles of catheter management and the nurse’s responsibility towards the catheterized patient.
Aim of the study:
This study aims to assess nurse’s practice for urinary catheterization and catheter care for patients with long term urinary catheter at the intensive care units and surgical urology department through the following:
1- Assessment of nurses’ knowledge about management of patients with urinary catheter.
2- Assessment of nurses’ practice in caring for patients with urinary catheter.
Research questions:-
To fulfill the aim of this study, the following research questions were formulated;
1- How can nurse knowledge affect the management of patients with urinary catheters in intensive care units and surgical urology department?
2- What is the relationship between nurses’ practice and occurrence of urinary catheter complication?
Subject and methods:
Research setting:
The study was carried out at the intensive care units and surgical urology department, Benha University Hospitals.
Subject
All available nurses working in the intensive care units and surgical urology department at Benha University Hospitals.
Tools
1-Questionnaire sheet: Will be designed to assess the existing knowledge of nurses in relation to the key components of indwelling catheter insertion and catheter care. It will be filled by nurses.
2-Observational checklist: will be designed to assess practices of nurses in the care for patients with urinary catheter.
Results
The results of the study showed that:
- The majority of nurses’ age were at the age of (<25 years).
- In relation to level of education, it was found that the majority of nurses were having Diploma in Nursing.
- As regard to marital status, the present study indicated that all of them were married
- According to years of experience, it was found that 48% had <5 years.
- Regarding training program, it was found that all nurses had no training program.
- There was lack of nurses’ knowledge with regard to patient catheterization and catheter care.
- There was a lack of practice regarding the procedure of urinary catheterization and a lack of quality of care for the patient with urinary catheter.
- There was no significant relation in nurses’ knowledge regarding information of urinary catheter care and practice.
- There was no significant relation between experience and practice.
- There was no statistical significant relation among knowledge, years of experience, practice, age and level of education.
- There was no correlation between ages, years of experience with total knowledge score and practice score.
Conclusion
It was noticed that no statistical significant difference was found among knowledge, years of experience, practice, age and level of education.
There is no difference in nursing performance about urinary catheter care among nursing staff.
There were gaps in nurses knowledge (e.g indications of urinary catheter, the problem of infection, catheter selection and its size, urinary catheter leakage, encrustation, catheter blockage and inadvertent catheter removal.) that affect the management of patients with urinary catheter, also there was a higher significant relation between nurses practice and occurrence of catheter complication( e.g infection, urethral trauma and restraint of the patient).
The most important recommendations are:
1. Nurses who take care of catheters should be given periodic in-service training stressing the correct techniques and potential complications of urinary catheterization.
2. Providing the knowledge and technique necessary for proper urinary management to all nurses.
3. Providing chance for a variety of professionals to interact with each other.
4. Demonstrating methods to diagnose and cope with urinary tract infection.
5. Demonstrating effective coordination between nurses and urologists.
6. Preparing guidelines on urinary management.
7. Preparing an information source for urinary management.
