The study was conducted through assessment of the completeness of 146 medical records for patients admitted to oncology unit. Descriptive analysis of the results was done and summation of composite medical records scores is done. Also, there is assessment of structure and functions of medical record department through assessment checklist. New forms for medical record were designed to ensure complete, consistent information and easy in filing and analysis through new computerized program. This program was designed for storage and retrieval of information and analysis of some of important items.
