Abstract

Chronic kidney disease (CKD) is a major public health problem that often goes unrecognized until late-stage disease. It is a growing problem in Egypt associated with an increase in the prevalence of dialysis patients.
The term quality of life is used to evaluate the general well-being of individuals and societies. HRQoL is considered a subset of quality of life (QoL), which is defined as ”the absence of sickness and disease, and a state of physical, mental, and social well-being. For the elderly, HRQoL is especially important because they may have multiple medical diagnoses and functionalز impairments that limit their independence and ability to engage in life activities. An important aim of research into HRQoL in this age group is to enable older people to maintain their mobility, independence, their active contribution to society, and to respond effectively to the challenges.
Functional status, an important domain of QOL refers to the ability to perform daily activities required to meet basic self-care needs and to maintain health and well being.
This study was conducted to assess the quality of life, activity of daily living, and Instrumental activity of daily living in elderly on chronic hemodialysis treatment.
This study is a case-control study performed on 90 elderly 60 years and older, male and female they were divided into three groups:
1- Group I: A sample of 30 elderly of matched age and sex, not suffering from renal disease.
2-Group II: A sample of 30 elderly of matched age and sex with renal failure under conservative treatment.
3- Group III: A sample of 30 elderly of matched age and sex with end stage renal disease on chronic hemodialysis.
All samples were recruited from the inpatient ward and outpatient clinics of Ain Shams University Hospital.
Every subject were subjected to comprehensive geriatric assessment, including personal history, past medical history, assessment of cognitive function using minimental state examination (MMSE), assessment of mood using geriatric depression scale (GDS 15), assessment of function using the Katz activities of daily living (ADL) and the Lawton Brody Instrumental activities of daily living (IADL) and assessment of health-related quality of life by the short form 36 health survey (SF36) and Karnofsky scale.
This study showed that quality of life is greatly affected among cases with chronic kidney disease, and that quality of life was affected more among cases on chronic hemodialysis.
Also this study showed that cases on chronic hemodialysis had lower performance in ADL (in bathing) and IADL (in shopping, food preparation, house keeping and usage of transportation).
