Abstract
Angiogenesis is involved in many ocular diseases. Many cytokines have been implicated in angiogenesis including (VEGF).Thus therapies that inhibit the effect of VEGF represents a new frontier in treatment of neovascular diseases.
Intravitreal injection of VEGF antibody exhibits great success in management of the following ocular diseases:
1-Choroidal neovascularization secondary to age related macular degeneration :
Intravitreal injection of VEGF antibody in patient with CNV leads to regression of neovascular membrane ,decrease in retinal thickness,resolution of macular edema and subsequently improved visual acuity.the effects of the injection can be followed up by fluorescein angiography and OCT.
2-Proliferative diabetic retinopathy complicated by vitreous hemorrhage:
In such cases VEGF is more useful as there is difficulty to use PRP due to hemorrhage.vitreous hemorrhage clears within few days after injection .also retinal edema and visual acuity improved.
3-Macular edema in central retinal vein occlusion (CRVO)
The macular edema resolves gradually and visual acuity improves gradually.this form of therapy saves retinal tissue compared to the destructive laser therapy PRP.
4-Neovascular glaucoma:
In this type of glaucoma IVT injection of VEGF antibody leads to fading of the new vessels and subsequently decrease in IOP.
It is observed that rubeosis of the iris has been completely disappeared after single injection.
5-Refractory pseudophakic cystoid macular edema:
This type of macular edema has been resolved in many cases after IVT of VEGF antibody .It is more save compared to steroids which is used to treat such cases .As steroids by different routs of administration carry the risk of elevating IOP.
6-VEGF antibody also used in revision of encapsulated filtering bleb by needling.
