Abstract

Coronary artery disease is the ultimate cause of death in more than half of diabetic patients and frequently manifests itself silently and prematurely.Patients with type 2 diabetes have a two-to-three fold increase in risk of early death from coronary artery disease.Coronary artery disease is often asymptomatic in type 2 DM and may present with sudden death, myocardial infarction, arrythmias or heart failure. Early detection of silent myocardial ischemia plays an important role in improving morbidity and mortality in diabetic patients.Many studies of silent ischemia have examined the painless ischemic response by analyzing ST depression during graded exercise testing or ambulatory Holter ECG monitoring. Although in many studies transient ST segment depression represents myocardial ischemia.The aim of this study was to evaluate the presence of silent myocardial ischemia in type 2 DM with no symptoms of ischemic heart disease, and to detect idependent risk factors for silent ischemia in this group of patients.This study was done on 30 patients,more than 35 years old, with type 2 DM for more than 2 years, who had no symptoms of ischemic heart disease.We excluded patients who are known to be ischemic, or on anti anginal treatment, patients with severe hypertension or malignant arrythmias and patients with organic heart diseases .
