Abstract

The topic of cervical radiculopathy has been discussed in the neurosurgical literature for decades. The posterior approach preceded the anterior approach by two decades, though the pendulum did swing away in favor for the latter approach over the last thirty years, the posterior approach remains a safe, cheap, and effective technique.
This prospective study was designed to evaluate and describe the posterior approach for the treatment of cervical radiculopathy.
This study included fifty patients all selected from those attending neurosurgery department Gamal Abd Alnasser insurance hospital, all were operated by the same surgical team using the same technique.
Of our fifty patients 43 were males while 7 females, with a mean age of 41.34 +/-8.16 years. The mean duration of complaints was 3.42+/-1.84 months. Brachialgia was the dominant complaint either alone or associated with other complaint(s). C7 was the most affected root followed by C6 then C5. Pure disc was the most common pathology followed osteophyte then dicso osteophytic complex. 82% of our patients showed excellent results, 18% showed good results. The best results were obtained in cases of pure disc pathology at C6-7 level, while the higher we go the less favorable results get obtained.
The posterior approach is a safe, reliable, cheap, effective technique. We are not here to fuel the debate about which approach is better we are only trying to say that this elegant, familiar and rewarding technique deserves a fair chair and is not to be neglected.
