Abstract

Sleep disordered breathing (SDB) remains more prevalent in CHF patients than in the general population despite advances in therapy. Patients with CHF might present decreased strength of the inspiratory muscles, which are currently recognized as factors implicated in their limited exercise response and quality of life as well as in their poor prognosis. The measurement of inspiratory occlusion pressure at 100 ms (P0.1) is thought to reflect the entire neural output of the respiratory centre which is hypothesized to be affected in patients with LVHF. The aim of this study was to study sleep disordered breathing, respiratory muscle strength and respiratory neural drive in patients with compensated heart failure. Fifty adult patients with documented LVEF ≤ 45% and stable medical condition and on optimal medical therapy were submitted to complete history, clinical examination, pulmonary function tests and polysomnography. According to the polysomnographic results, patients were divided into two groups one for those without SDB [12 (24%) patients] and the other for those with SDB [38 (76%) patients], defined by an apnea- hypopnea index ≥ 10 events/ h. The later group was subdivided into two groups one for those with obstructive sleep apnea (16 patients = 32% of the total patients and 42% of SDB group) and the other for those with central sleep apnea (22 patients = 44% of the total studied patients and 58% of SDB group). Males constituted 56% of all the studied patients. In patients with SDB 65% of the patients were male and 35% were females while in the group without SDB 25% of patients were males and 75% were females. The mean BMI for all the studied patients was 24.9 ± 3. For patients with SDB, the BMI was higher than for patients without SDB and in patients with OSA compared to those with CSA.
