Abstract

Hip fractures among the elderly constitute an increasing burden on healthcare resources with significant post-fracture disability, morbidity and mortality.
The management of displaced intracapsular femoral neck fracture in physiologically active patients remains a matter of hot debate, and thus is called ‘unresolved fracture’. A femoral neck fracture should be fixed surgically, unless the patient is very ill. The type of surgical procedure used depends on the type of fracture according to Garden’s classification: internal fixation is recommended for an undisplaced fracture, hemiarthroplasty (HA) or total hip arthroplasty (THA) is recommended for elderly patients with displaced fracture, and open reduction and internal fixation is preferred for younger patients.
The aim of this study was to evaluate the early results of cemented bipolar hemiarthroplasty in the treatment of displaced intracapsular femoral neck fractures in elderly patients.
This study included 23 patients. The mean age in the study was 66.5 years ranging from 61 years to 73 years. It included 8 males(34.8%) and 15 females(65.2%). The mechanism of injury was mainly simple fall and only two patients sustained RTA. The right side was affected in 9 cases while the left side was affected in 14 cases. The mean ASA score was 1.6. All the patients were osteopenic with the mean Singh’s index 3.6. All the patients were community ambulators, active, and independent. The time lapse between admission and surgery was between 2 and 8 days. The patients were followed up in a period of one year using Harris Hip Score as a method of clinical assessment and radiographs to assess the radiological results.
Four patients had postoperative complications. One had superficial infection, one had early deep infection, one had one attack of dislocation and one had DVT. The patient who had early deep infection had also acetabular erosion with superior and medial migration of the prosthesis. Radiologically no cases had femoral stem loosening or heterotopic ossification.
The results of the study showed that 18 patients were satisfactory(78.3%) and 5 patients were unsatisfactory(21.7%), and the mean final Harris Hip Score was 84.4 points.
The final results were affected by the age as the patients with younger age had better results than the patients with older age. The final results were affected by sex with better results in males than in females. Also the final results were affected by the efficiency of the abductor mechanism because the patients with satisfactory results had negative Trendelenberg test while the patients with unsatisfactory results had positive Trendelenberg test. Also the patients who had positive Trendelenberg test used external walking aids more frequently.
The final results were affected by the presence of postoperative complications as the patients who had postoperative complications had worse results than the patients who had no complications.
The final results were not affected by the mode of trauma, the side affected, the medical comorbidities, the occupation, the time lag to surgery, the position of the stem or the cement mantle grading.
