Abstract

To validate whether DTI of lateral tricuspid annulus provides a clinically reliable method to determine pulmonary vascular resistance (PVR) in patients with pulmonary hypertension secondary to congenital heart disease (CHD).
Methods
DTI of lateral tricuspid annulus and right heart catheterization were performed in 50 patients with congenital heart disease and pulmonary hypertension within 24 hrs of each other.
The PVR measured by DTI of lateral tricuspid annulus was then correlated with the invasive PVR measurements by cardiac catheter using regression analysis.
The equation PVRECHO= 3698-1227x In (tSm) was compared with invasive PVR measurements using Bland-Altman analysis and t-test for paired samples.
Results
As calculated by DTI, PVR correlated well (p value < 0.001) with the invasive PVR measurements.
The Bland-Altman analysis between PVR obtained invasively and that obtained by DTI of lateral tricuspid annulus using the equation: PVRECHO= 3698-1227x In (tSm), showed non satisfactory limits of agreement (mean 162.3±643.2(SD) dyne.sec/cm5).other models gave more favorable results were generated but still of limited clinical value.
Conclusion
DTI of lateral tricuspid annulus peak systolic motion (tSm) may provide a reliable non invasive method to evaluate PVR in patients with PH secondary to CHD.
