Abstract

Significant left main coronary artery stenosis is a well-accepted indication for coronary artery bypass grafting (CABG). However, waiting lists for surgery may result with subsequent increase in the risk of MACE during the waiting time.
In this study, we aimed to determine the incidence of MACE and to investigate the predictors of these events in patients with LMCA disease before CABG.
A total of 22 patients with significant LMCA stenosis (≥50%) were included in the study. Detailed demographic, clinical, and angiographic data were obtained for each patient. In order to monitor the occurrence of serious cardiovascular events after coronary angiography, patients were followed up for at least six months or till CABG.
CABG was associated with reduced (MACE) which occurred in 45.5% of the study group during the follow up period, of whom two patients died (9.1%). Hospital admission for acute coronary syndromes was a significant predictor of cardiac events among the patients awaiting CABG (ACS in the eventful group, 90%; ACS in the uneventful group, 33.3%, P = 0.005). The presence of PVD and CVS were also associated with serious cardiac events. whereas, risk factors for coronary artery disease, LV ejection fraction, severity of LMCA, site of LMCA stenosis, RCA stenosis, presence of collaterals between LMCA and RCA, and waiting time for CABG did not influence the preoperative outcomes among patients with LMCA stenosis.
from the results of this study, we conclude that patients with significant LMCA stenosis who had acute coronary syndrome, and those with other peripheral vascular disease and CVS are considered high risk group for MACE, and we recommend that such patients should be scheduled for emergency CABG.
