Abstract

Chronic periodontitis is the most frequently occurring form of periodontitis resulting in inflammation within the teeth supporting tissues, progressive attachment loss and bone loss. Diabetes is a chronic metabolic disorder with several complications including periodontitis. Recently data showed that circulating TNF-α is associated with more severe periodontitis, TNF-α has been reported to play a key role in the pathogenesis of type two diabetes.
Aim of the study: The aim of this study was to investigate the influence of chronic periodontitis severity on circulating levels of TNF-α in patients with type 2 diabetes mellitus.
Materials and Methods: This study was conducted on forty chronic periodontitis patients suffering from type two diabetes mellitus (28 females and 12 males), subjects ranging in age from 40 to 60 years. They were divided into two groups:
group Ι: included twenty chronic periodontitis patients with controlled type 2 diabetes mellitus, they were treated with scaling and root planning.
group П: included twenty chronic periodontitis patients with uncontrolled type 2 diabetes mellitus, they were treated with scaling and root planning. For both groups, prior to such treatment, a venous blood sample was drawn from the patient and another venous sample was drawn after consequent four follow-up visits for these patients.
Results: Clinical parameters(PD, GI, PI) showed significant reduction when compared to pre-treatment levels, however, AL was significantly decreased only in uncontrolled diabetics group to pre-treatment levels. There was significant increase in blood glucose level in uncontrolled diabetic group when compared to controlled diabetic group before and after treatment, however, there was no significant difference of TNF-α between both groups before and after treatment.
Conclusion: Circulating TNF-α is not reduced following periodontal treatment in subjects with type two diabetes mellitus, this is due to the positive correlation between the Blood glucose level and the circulating TNF—α.
