Abstract

from the present study the following can be concluded :
- Aggressive benign bone tumors are those which can progressively destroy bone with high local recurrence rate and sometimes metastatic lesions.
- Aggressive benign bone tumors include giant cell tumor, aneurysmal bone cyst, chondoblastoma, chondromyxoid fibroma and osteoblastoma.
- Treatment of aggressive benign bone tumor represent a challenge with much controvers.
- Treatment includes extended currettage in combination with adjuvant therapy to decrease the risk of local recurrence which is the most common complication.
- There are many adjuvant therapies used in the literature including:
phenol, hydrogen perioxide, liquid nitrogen and most recently the use of cyotoxic drugs. As well as bone graft and bone cement which are used to fill the resultant defect.
- The most common complication when liquid nitrogan is used as adjuvant therapy is pathological fracture,which can be miniminized by
sealing the base of the window with wet Gelfoam.
- The aim of this study is to determine if adjuvant phenol or liquid nitrogen improve the recurrence rate of aggressive benign bone tumors as compared to standard treatment.
- Thirty patients with aggressive benign bone were included in the study
.Phenol was used in fifteen cases while liquid
- The complications were found in 4(13.3%) of cases ,with overall local recurrence occurred in 3(10%) of cases,1 case (6.7%)of whome in which liquid nitrogen was used and in 2(13.3%)cases of whome in which phenol was used Fracture occured in 1(3.3%)case which was treated with liquid nitrogen.Additionally one case of those with local recurrence was complaining of nerve palsy.The mean functional score was 93.5 ± 2.4%.
- So, we recommend the use of phenol or liquid nitrogen as adjuvant therapy in the management of aggressive benign bone tumours following extensive curettage.
- We found that both phenol and liquid nitrogen have nearly similar complications rate.
- The liquid nitrogen has relatively lower local recurrence rate as compared with phenol however it carries the risk of pathological fracture more than phenol.
- Phenol has higher rate of never palsy.
