Abstract

Apoptosis is a tightly regulated process of genetically programmed cell death. The major feature of non-melanoma skin cancer is their resistance to apoptosis. The present study investigated the role of some apoptosis regulatory proteins in BCC, SCC and AK. The apoptotic cells were detected by TUNEL staining. The study was conducted on 29 cases of skin carcinoma. 14 BCC cases, 10 SCC cases and five A.K cases. Full history taking, general, local examination of the lesions and photographing were done. The diagnosis was confirmed by H&E histopathological examination. Then, specimens were analyzed by immunohistochemistry using P53, Bcl-2, Bcl-xl, Caspase-3 and Fas antibodies as well as TUNEL technique to detect apoptotic cells. Results showed that P53 was expressed in 57.1 % of BCC, 80% of SCC and 80 % of AK cases, while its immunoreactivity was observed in 10% of normal skin. Bcl-2 was expressed in 85.7% of BCC and in 80% of AK cases whereas it was not detected in any SCC. Immunopositivity for Bcl-xl was detected in 72.6 % of BCC, 60% of SCC and 60 % of AK. Caspase 3 was also expressed in 78.6 % of BCC, in 60% of SCC and in 40% of AK. Positive Fas staining was observed in all examined SCC and AK cases (100%) while none of the BCC specimens showed positive Fas expression. By TUNEL staining, The mean Apoptotic index for BCC was 0.78 ± 0.07, while the mean Apoptotic index for SCC was 1.28± 0.45, and the mean Apoptotic index for AK was 0.3 ± 0.3. The apoptotic index of SCC and BCC was significantly higher than that of actinic keratosis.
