Abstract Fast track surgery program is a new set of protocols that is applied to the patient in the peri-operative period to fasten the recovery and decrease the convalescence and thus improving the surgical outcome. It is also named as enhanced recover after surgery (ERAS) program. It was pioneered in by gastrointestinal surgeon called Henrik Kehlet from Copenhagen in 1990.later on several groups around the world confirmed the beneficial results of this program. The Components of fast track surgery program can be summarized in: 1- Preoperative elements that include, patient education and counseling, optimization of the physical conditions of the patient, optimization of nutritional status, alcohol and smoking cessation, avoidance of bowel preparation and premedications. 2- Intraoperative elements that include, fast track anaesthetic techniques, use of minimally invasive surgical techniques and avoidance of routine use of nasogastric tube and drain. 3- Postoperative elements that include, proper management of postoperative pain, nausea, vomiting and ileus, early oral feeding, early removal of urinary catheters and early ambulation. We should realize that discharge criteria with fast track program are that with the conventional care but achieved earlier. Application of fast track surgery program requires cooperation among the patient, surgeon, anesthetist and nurse. Fast track surgery program can be widely applied in different surgical procedures as colorectal, vascular, orthopedic, urologic and gynecological surgery. That will result in decreasing the length of hospital stay, improving recovery and surgical outcome. Also fast track program has real economic benefits resulting from this short hospital stay without increasing the risk of complications. |
