Abstract

Aim of work: The aim of our work is to study the prevalence of type 2 diabetes mellitus in patients with chronic hepatitis C virus infection in Dakahlia Governorate.
Patients and methods: This study comprised 700 subjects with chronic hepatitis C virus infection (485were males and 215 were females) and 200 hepatitis C negative subjects as a control group (116 were males and 84 were females) the patients were selected from the central hospitals in 4 centers from Dakahlia governorate (Mansoura, Manzala, Dekernis, Minyat El-Nasr). The following patients were excluded (Exclusion criteria): Patients with diabetes mellitus secondary to other endocrinal diseases as cushing, acromegaly…..etc, Patients taking diabetogenic drugs as corticosteroid and patients with type I diabetes mellitus. The patients and the control group were subjected to the following assessment: Medical history including: Age, Residence, Duration of treatment for both hepatitis C infection and for diabetes mellitus. Clinical examination including , General examination with special stress on pulse, blood pressure, pallor, jaundice, palmer erythema, spider navi, nutritional status, Oedema lower limb and other general manifestation of chronic liver disease and Abdominal examination for size of the liver, size of the spleen, ascitis and other manifestations of liver disease. Laboratory investigation including, Fasting and post prandial blood sugar, Test for HCV antibody, Test for HBSAg, Complete blood picture, Liver function test including liver enzymes, serum albumin, prothrombin time, Serum triglycerides, Serum cholesterol level. Abdominal ultrasound and liver biopsy.
Results: We found that the prevalence of type 2 diabetes mellitus in chronic hepatitis C infection is 16.3% (114 patients from 700 HCV patients were found to have type 2 diabetes). We also found that serum cholesterol and triglycerides were comparatively lower in the hepatitis C virus positive patients than the control group. We found that hemoglobin level and platelet count were significantly lower in the patients than the control group. There is significant increase in serum ALT than the control group. There is also significant positive correlation between the metavir fibrosis stage and fasting blood glucose level. There is significant increase in the age and the duration of HCV infection in the diabetic group than the non diabetic group.
Conclusions: Chronic hepatitis C is a major health problem as it causes chronic liver disease and its association with the development of type 2 diabetes mellitus and insulin resistance and its complications.
