Abstract

Living-donor liver transplantation is a remarkable effective lifesaving procedure for selected patients with end-stage liver disease . The demand for living-donor liver transplantation will probably increase over the next few years because of the critical shortage of donor organs and the favorable initial results . With an expert surgical team and appropriate selection of recipients and donors , the benefits to the recipients and donors, the benefits to the recipient outweigh the risks to the donor . Long-term success of living-donor liver transplantation will require careful , thoughtful application of this new procedure .
Once a patient has been medically accepted as a candidate for LDLT by the transplant team , he or she may accept volunteer donors for evaluation . The manner in which donors are selected for donation is important to ensure that their participation is entirely voluntary . To be considered for initial evaluation , the donor must have the following characteristics : be between the ages of 18 and 65 , have the identical blood type as the donor or a blood type that is compatible and have no significant medical problems .
Although most donors undergo successful right hepatectomy without any problems due to great progress in surgical and anesthetic management of liver transplantation , a small proportion develop complications . The most common complications in the donor are biliary problems , which occur in approximately 5% of donors . Between 9% and 19% of donors develop other complications related to undergoing a major abdominal surgery including wound infection , small bowel obstruction , and incisional hernia . The geatest concern for donor safety is the risk of donor death . The exact risk of donor death is uncertain , because no formal worldwide registry of all LDLT cases exists . However , in the reported cases of LDLT , there have been two donor deaths out of 806 (0.25%) donor hepatectomies . Concern has been expressed by transplant physicians related to donor mortality As a result , general guidelines have been formulated regarding donor informed consent , medical suitability , psychosocial suitability , and criteria for transplant centers performing LDLT .
