Abstract Discharge planning has a very important role in discharge of ostomates patients to reduce complication and help them to adapt on new lifestyle after stoma. The main objective; of this study was to determine the effect of discharge planning for patients with stoma (colostomy or illeostomy) on their lifestyle pattern. The research hypothesis; there will be a positive relationship between the discharge planning for patients with stoma and lifestyle pattern. The study sample consisted of (60) patients, thirty in study and thirty in control groups from both sexes and she/he has new colostomy or illeostomy on El-Demerdach Hospital and National Cancer Institute in Surgical Departments, and Outpatient Clinic for follow-up. The tools of data collection was patient assessment sheet; to assess patient condition, which included physical and psychological with sociodemographic data; discharge planning booklet was given for study group only; patient questionnaire sheet to assess patient knowledge; observation checklist for stoma care and change appliance; and lifestyle questionnaire for patients with having an ostomy. The results revealed that there was no statistically significant difference between both groups under study as regard patient assessment, and highly statistical significant difference between study and control group in relation to patient‘s knowledge, performance and lifestyle in following up phase included (physical and psychological conditions); while no statistical significant difference in relation to social and spiritual conditions. In conclusion; Improve in patient knowledge, practice, and lifestyle after giving them discharge planning. The study recommended that application of model discharge planning ”booklet” before discharge to patients can help them to adapt an alteration in their lifestyle and improve quality of life.
