Abstract

Jaundice is the most common condition that requires medical attention in newborns. Some infants, serum bilirubin levels may excessively rise, which can be cause for concern because unconjugated bilirubin is neurotoxic and can cause death in newborns and lifelong neurologic sequelae in infants who survive
Neonatal jaundice is effectively treated with phototherapy . The goal of photherapy is to lower the concentration of circulating bilirubin or keep it from increasing. Phototherapy achieves this by using light energy to change the shape and structure of bilirubin, converting it to molecules that can be excreted even when normal conjugation is deficient.
This study was conducted over a period of 12 month from 1/4/2007 to 1/4/2008in Omm El Atebba hospital (Neonatal Intensive Care unit) in Giza Governorate .This hospital is a specialized paediatric governmental hospital .This study was comprised of three stages
Screening Stage :During which preparation and classification of the sample into cases (study group n=41 ).and (control groups n=38).
Intervention Stage:As cases will use single phototherapy with white reflecting curtains
Follow up Stage: During which efficacy of the usage of the phototherapy with curtains was assessed also the safety of the method clinically, and loboratory was assessed. All cases of the study and control group were subjected to history taking (prenatal ,natal, postnatal and family history),and full clinical examination including anthropometric measurement(weight, height and head circumference).and Safety of both methods will be assessed and compared by monitoring the body temperature hydration status Skin problems, and gastrointestinal problems Decision to stop phototherapy were made according to guidelines from clinical practice guidelines given by the Egyption Clinical Practice Guidelines for Management of Neonatal jaundice in term newborn approved by(Ministry of Health Population Arab Republic of Egypt,2000) Investigation were included Serum bilirubin. line and at 0,4,10,24 and 48 hours of initiation,Complete blood picture,Creactive protein, reticulocytic count , coombs test Blood group and rh type for the the patient and patients mother,and total serum calcium level before discharge.
The material for the white textile curtains which used in the study was one curtain 100cmX50cm and two curtains 50cmX50cm cotton made hanging from the three sides of the phototherapy unit .the material of the white curtains that are readily available in most neonatal intensive care units which is cotton made.
All units used in the study were serviced just before the study began and were maintained in optimal working condition for the whole duration of the study .The distance between the phototherapy unit and the baby was standardized at 50 cm.
The result of that study was the mean (standard deviation)decrease in total serum billirubin levels after 4 hours of phototherapy was significantly (p>0.001) higher in the study group (1.097 mg/dl) than in the control group(0.19 mg/dl)Also Kaplan –Maier statistics shows the probability of the duration of phototherapy among the study group52 hours and the control group90 hours .No difference in adverse events was noted in terms of hyperthermia or hypothermia, weight loss,rash, loose stools .
