Abstract

Temporal lobe epilepsy (TLE) is defined as recurrent unprovoked seizures that originate from the medial (mesial) or lateral temporal lobe. Intractability is a common issue in TLE, of which the most common cause is mesial temporal sclerosis and reduced hippocampal volume as measured by specialized MR volumetric studies. The presence of hippocampal atrophy by MRvolumetrics in patients with intractable TLE is a reliable predictor of postsurgical seizure-freedom with minimal, if any, dosages of AEDs The aim of this study is to analyze cortical thickness in the temporal lobe structures by using MRI-volumetrics and to correlate the results with intractability of seizures. This study was carried out on 34 patients with TLE in addition to 20 normal subjects as a control group. The patients were divided into 4 groups according to their clinical and EEG data: controlled right temporal lobe epilepsy (RTLE) ; intractable RTLE; controlled left temporal lobe epilepsy (LTLE) ; and intractable LTLE groups. The subjects have been evaluated by clinical history and examination; EEG; and MRI hippocampal volumetric studies.
