Abstract Off-pump coronary artery bypass grafting (OPCAB) surgery as an alternative method for surgical coronary artery revascularization is still discussed controversially regarding its benefits compared with conventional coronary artery bypass grafting (CCAB) surgery. The aim of this study is to compare the incidence of perioperative myocardial infarction between on-pump and off-pump CABG. Methods, between August 2006 to May 2008, 60 patients had coronary artery bypass grafting (group A: 30 without CPB; group B: 30 with CPB). Incidence of PMI (documented by ECG changes, CK-MB and Troponin T level and echocardiography) was recorded. Results, The mean number of grafts was 1.4 ±0.73 grafts in off pump group and 2.7 ±0.83 grafts in on pump group. No significant difference found in the incidence of PMI between both groups. . Cardiac troponin T and Ck-MB show higher levels in the on pump group at hours 4, 12, 24 and 48 than in the off pump group. However, this does not indicate MI. Patients undergone off pump had significantly shorter time of ventilation when compared to on-pump patients, less number of patients requiring inotropic support, less ICU and hospital stay. Conclusions, no significant difference was found in the incidence of PMI between both groups. Off-pump techniques became one of the alternatives that should be available and mastered for a coronary surgeon. Limitation of the study is due to small number and non randomization of patients among both groups. |
