Abstract
This study aimed at developing a database for glomerulonephritis patients in the nephrology clinic, pediatric hospital, Ain Shams University attending the clinic from 1/2005 to 1/2011. A trial to improve the completeness, storage and retrieval of medical records was carried out by development of computerized program.
The study was conducted through assessment of the completeness of 347 medical records for patients attending pediatric nephrology clinic. Descriptive analysis of the results was done. New forms for medical record were designed to ensure complete, consistent information and easy in filing and analysis through new computerized program. This program was designed for storage and retrieval of information and analysis of some of important items.
Assessment of completeness of medical records revealed that the contents of records have many deficiencies. The completeness percentage of MR contents ranged from (100 to 0.008%). The greatest item in completeness after name, age and gender (100%) was the treatment (92%); and the lowest score was for the date of birth (7.5%) and height(.008%) of all reviewed records. The contents of records were arranged in order, but not reviewed for completeness of documentation.
After assessment phase, designing and development of new medical record forms were done to ensure simple, easy and effective way of entrance and retrieval of medical data, and newly designed form (summary sheet) for regular revision of the patient important information.
A computer program is designed for the manipulation and analysis of data in medical records. And it allows easy retrieval of patient information.
The objectives were achieved through:
1- Assessment of current status of glomerulonephritis patients in nephrology clinic, pediatric department in Ain Shams University.
2- The development of models for data collection and patient follow-up.
3- Establishment of the necessary programs and database for glomerulonephritis patient attending the nephrology clinic, pediatric hospital, Ain shams university.
4- Revision of the available patient’s information and entry of this information to the prepared computer program.
5- Analysis of the data available in the patient based database.
We hope that the designed database will be able to help improve the performance of health service provided to these patients.
