Abstract

This study is designed to evaluate the role of complement C5 both intrapulmonary and in the serum in atopic asthma .
The study was conducted on fifty patients with atopic bronchial asthma selected on the base of high serum total IgE level being higher than 100Iu /dl and presence of recurrent attacks of cough , wheezes and chest tightness that were reversible spontaneously or with bronchodilator therapy. The patients were selected from Elfashn chest hospital Banisuef , governate. The asthmatic group was classified according to PEFR, signs and symptoms into mild, moderate and severe groups.
Twenty five healthy subjects were selected as a control Group.
All patients and controls enrolled in this study were subjected to the followings :
1-Detailed history
2-Clinical examination
3-Peak expiratory flow rate.
4-Routine laboratory investigation including:
1-CBC
2 -Liver function tests: ALT AST Total bilirubin
3 - Renal function tests
Serum urea
Serum creatinine
4 -Stool analysis
5 -Urine analysis.
5-Chest radiography.
6-Erythrocyte Sedimentation Rate.
7-Specific investigations:
- Serum total immunoglobulin E(IgE)
-Level of complement C5 in serum
- Level of complement C5 in sputum
We demonstrated highly significant statistical difference between groups regarding IgE level being higher in asthmatic group with range 100.9-640 mg/dl and mean 235.1 and significant increase in serum IgE in severe group in comparison with moderate group .
In our study we proved highly statistical difference (P< 0.0001) between patients and control regarding the mean values of C5 levels in serum being higher in the asthmatic group.And also highly significant increase in serum C5 in severe group in comparison with mild group (P2<0.0001) and moderate group(P3<0.0001).
Also in our study we recognize that there is highly statistical difference between patients and control regarding the mean values of C5 levels in sputum being higher in the asthmatic group and highly significant increase in sputum C5 in severe group in comparison with mild group and moderate group.
In our study we proved also a highly significant correlation between Serum C5 and IgE level (P<0.003) in asthmatic group.
Also our results demonstrated no significant correlation between Serum C5 level and Sputum C5 (P<0.111) in asthmatic group supporting a major contribution of local complement production .
