Abstract

Burns of the hand are common injuries caused by contact with various types of thermal or radiant energy or caustic chemicals (Peacock, 1977).
Severe burn injury of the hand frequently results in significant physical limitations and emotional problems because the patient is no longer able to perform the activities of daily living and functions at school or work (Salisbury and Wright, 1982).
The treatment of burned hand has been one of the most controversial topics in modern surgical burn therapy (Habal, 1978).
A variety of approaches has been advocated for the treatment of burns of the hand; conservative treatment with topical antimicrobials and early surgical excision and immediate grafting (First et aL, 1985).
There are two different types of burn wound excision; fascial excision involves removal of the burn and all underlying fat to the level of investing muscle fascia which is performed with scalpel or electrocautery, the other is tangential excision (Heimbach, 1987).
The term tangential excision implies excision of the necrotic surface of a burn, taking repeated slices parallel to the skin surface using a skin graft knife (Jackson and Stone, 1972).
Early excision and grafting of the burned hand have gradually overtaken conventional therapy with topical antibiotics (Baux et a]., 1987).
Hunt et al. (1979); Engrave et a]. (1983 and 1986) found that early excision and grafting of the burned hand permits early healing, good function and cosmetic results and shortened hospitalization time.
