Abstract

This study was performed on fifty patients with chronic HCV attending Mansoura general hospital outpatient clinic during the period from December 2011 to October 2012, and 25 healthy volunteers were enrolled as control group. All patients and controls were classified into 2 groups.
Group Ι:- HCV paitents were categorized into 2 groups:-
Group 1A:-chronic HCV patients with insulin resistance Group 1B:- chronic HCV patients without insulin resistance Group ΙΙ:-Twenty five healthy volunteers of matched age, sex and body mass index.
All participants were subjected to detailed history taking, thorough clinical examination as well as laboratory investigations (complete blood picture, liver function tests, kidney function tests, hepatitis markers,
Fasting blood sugar, fasting insulin, HOMA-IR index, abdominal ultrasonography and Liver biopsy ”done for patients only”).
Statistical analysis of the results of the present study revealed the following points:
- No significant difference between the studied groups as regard age, BMI and sex distribution.
- As regard platelets count it was higher in group 2 than group1A and group 1B.
- as regard ALT and AST values, it showed a high significant difference between group1A and control group, a high significant difference between group1B and control group and no significant difference between group1A and group1B.
- while there were no significant difference Between group1A,group1B and controls as regard Hb ,WBCs, Creatinine, Prothrombin time and Total Billirubin.
- As regard fasting blood glucose it was higher in group 1A than group1B and controls and no significant difference between group1B and controls.
- As regard fasting insulin it was higher in group 1A than group1B and controls and no significant difference between group1B and controls.
- As regard HOMA.IR it was higher in group 1A than group1B and controls and no significant difference between group1B and controls.
- No significant difference between group 1A and group 1B as regard PCR.
- The frequency of prediabetes in chronic non-diabetic HCV patients is 64%, which is statistically higher than control group.
- No significant difference between group 1A and group 1B as regard liver biopsy.
- No correlation between PCR and HOMA.IR value in group1A and in group1B.
- No correlation between liver fibrosis and HOMA.IR value in group1A and in group1B.
