Abstract

This study consists of 40 patients, divided into 3 groups, one group presented in ER by respiratory distress and diagnosed as pneumonia, second group presented in ER by respiratory distress and diagnosed as bronchiolitis, and third group is control group.
In this study we found increase in total leucocytes count and CRP with significant difference in cases of pneumonia than brochiolitis and this is may due to bacterial nature of infection in cases of pneumonia as brochiolitis is mainly viral nature and both group were anaemic and this already found in Egyptian people due to bad habit of feeding and low socioeconomic level, and we found tachycardia and hyperothermia with significance difference in cases of pneumonia than bronchiolitis this was due to severity of inflammation and bacterial nature in cases of pneumonia than bronchiolitis but in brochiolitis we found tachypnea with significant difference than pneumonia that was due to age difference between pneumonia group and bronchiolitis group as respiratory rate in low age normally higher than high age so tachypnea was not conclusive for bronchiolitis to be differentiated from pneumonia in this study, and we found increasing respiratory distress with significant difference in cases of pneumonia than brochiolitis and this was due to the more severe inflammatory process and reaction and aggressive organisms in cases of pneumonia than bronchiolitis.
we found O2 desaturation with significant difference in cases of pneumonia than brochiolitis due to aggressive nature and more tissue affection in cases of pneumonia than bronchiolitis and we found increasing level of BNP with significant difference in cases of pneumonia than brochiolitis and this was due to more sepsis produced by pneumonia.
we found lobar pneumonia represented 60% of pneumonia cases while bronchopneumonia presented in 40% of cases and the most common cause of lobar pneumonia is streptococcal pneumonia in pediatrics with no significant difference in level of BNP between lobar and brocho-pneumonia, we found positive correlation between BNP and each of temperature heart rate, respiratory rate, decrease O2 saturation, increasing respiratory distress in cases of pneumonia and this was not found with bronchiolitis, we found long hospital stay with significant difference in cases of pneumonia that brochiolitis with positive correlation with BNP in pneumonia, association of long stay in hospital and severity of respiratory distress and its correlation with high level of BNP is associated with more morbidity and risk of complication from pneumonia itself and from infection acquired from hospital.
