Abstract

RT has proven to introduce favourable effects on reverse ventricular remodeling either mechanical or electrical & overall mortality, this in turn reflects improvements in symptoms and sings of Heart Failure and reducing heart failure hospitalizations
Many studies were done to prove the favourable effect of CRT on mechanical and electrical remodeling, the incidence on arrythmias and the relation between electrical and mechanical remodeling.
Our study was done aiming to demonstrate the effect of CRT on electrical remolding we observed significant changes in both depolarization and repolarization parameters on surface ECG after CRT. Also there were significant differences between responders and non responders regarding changes in QRSD, QTd, and TPE. Furthermore, these changes were significantly correlated with changes in NYHA class and echocardiographic parameters. Changes in QRS duration (∆QRSD) and repolarization heterogeneity (∆QTd and ∆TPE) could predict response to CRT with high sensitivity and specificity. CRT responders showed more reduction in native QRS duration and measures of repolarization heterogeneity (QTd and TPE) than non responders. Apart from QT and QTc intervals, reverse electric remodeling was associated with reverse structural remodeling and clinical improvement.
