Abstract

T
he data of this study were collected from the records of Qus sanatorium during the years 2005-2009 and revealed the following.Total number of patients came to sanatorium were 9901of which 267 were tuberculous cases (2.705),the number of tuberculous male patients were 113 out of 267 with a percentage of 42.32%, while the total number of female patients were 154 patients out of 267 with a percentage of 57.68%.,according to age, it was shown that the peak incidence of tuberculosis occurred between 20-29 years of age (32.58%), the lowest incidence was in the age group between 1-9 years (1.5%) and above 60 years (8.24%), with zero percent below one year,the distribution of cases according to occupation showed that the percentage of tuberculosis was 29.59% among housewives,the unemployments come next with a percentage of 28.09%. The lowest inciddence was among old age and children 5.24% and farmers 6.74%, clasification of tuberculous patients according to their reference showed that, the number of patients who attended the sanatorium themselves was the highest (45.69%).
The common tuberculous cases that had been recorded during the years of study were pulmonary tuberculosis (58.43%). The extrapulmonary tuberculous cases were (41.57%) with no significnt statistical difference between 2005 and 2009 (P>0.05), the main presenting symptom of tuberculosis was cough 54.30%. The newly diagnosed tuberculous cases during the years of study were 95.88%. The retreated cases were 4.12%.. Far advanced letions constitute the highest percentage (57.69%), then moderately advanced letions with a percentage of (22.44%), while the minimal letions represented the lowest percentage (19.87%), according to direct smear examination, smear +ve cases were 117 out of 156 pulmonary tuberculosis (75%), represent (43.82%) of the total number tuberculous patients (267 cases),the most common form of tuberculosis that had been recorded during the years of study was pulmonary tuberculosis 58.43% (156 cases out of 267). The miliary tuberculosis was 2 cases with a percentage of.75% during the years of study. The total number of primary tuberculosis was 7 cases with a percentage of 2.62% of the total tuberculous cases, as regards the major complications affecting the studied tuberculous cases, the study revealed that core pulmonale 6 cases (2.25%) and bronchiectasis 5 cases (1.87%) were the common complications.
Most of extra thoracic cases 94 patients (35.20%) were tuberculous lymphadenitis (45.74%), followed bytuberculosis of the bone (20.21%), tuberclous entritisand tuberculous peritonitis (18.09%), pericardial effusion (4.26%), tuberculosis of the skin (4.26%), others (7.45%) according to tuberculin skin test,94.01% of the patients were tuberculin positive and only 5.99% were tuberculin negative, distribution of cases according to BCG vaccination demonstrated that 64.04% of the cases were not vaccinated and 35.96% were vaccinated.
Final situation of smear positive tuberculous patients were 50 cases (42.73%) cured out of 117 cases smear positive, 51 cases (43.59%) completed treatment, 6 cases (5.13%) died, 8 cases (6.90%) defaulted, two cases (1.72%) referred to other hospital and no treatment failure, final situation of smear negative tuberculous patients revealed that, 123 patients (82%) completed treatment, 5 patients (3.33%) died, 16 patients (10.67%) defaulted, 6 patients (4%) referred to other hospitals, and no treatment failure, the treatment outcome of tuberculous patients treated in the sanatorium after completing the treatment cource revealed that, the number of patients who have been cured were 50 patients (18.73%) out of 267 patients. The number of patients who had completed their cource of treatment were 174 patients (65.17%). The defaulters (treatment interrupted) were 24 patients (8.99%) out of 267. It was no treatment failure. There were 11 patients (4.12%) died.
On comparison between final outcome of smear negative and smear positive cases the study revealed that cure and complete treatment (37.83%) 101 cases for smear +ve cases and (46.07%) 123 cases for smear –ve cases, defaulters (3%) 8 cases and (6%) 16 cases for smear +ve and smear –ve respectively, (2.24%) 6 cases for died smear +ve and (1.87 %) 5 cases for died smear –ve, referring for other hospitals were (0.75%) 2 cases for smear +ve patients and (2.24%) 6 cases for smear –ve ,total number smear –ve (150 cases) of 56.18% ,total smear +ve (117) of 43.82%, table (23).
