Abstract

In the postpartum period, mothers face sudden change in their life - style and the transition from the wife’s role to the maternal role, responsible for children activities. So it may lead to many conflict and negative coping pattern. The effectiveness of maternal adaptation is based on many factors such as, physical, social, and psychological that may create negative adaptation during this period. This is may lead to many physical and psychological stress.
Nursing role focuses on assessing women’s ability to adapt to the physiologic and psychological changes occurring at this time.
Aim of the study:
To assess prim para mother’s coping strategies during early puerperium.
Research design:
A descriptive study design.
Setting:
This study was conducted at the postnatal ward in delivery unit of Ain Shams University Maternity Hospital.
Subjects:
A simple random sample, with the following criteria:
•primpara mothers with a normal pregnancy and delivery with or without episiotomy during first four twenty hours. After delivery, then after three days, and one week for follow up.
•Mother who had delivered a healthy baby (with normal ApGAR score-weight and without any congenital anomalies).
•Mother who had telephones to be easily contact during follow up visits.
Sample Size:
Total sample of 674 women that represents about 10% of total women admitted to the hospital for normal vaginal delivery at previously mentioned setting in year (2010-2011).
- Tools of data collection:
1- Structured interviewing questionnaire:
It was designed by the researcher, which includes:
→Used to assess the subject’s general characteristics data .
2- Coping strategy scale: (Fraster et al, 2003)
1st part: stressors assessment sheet which contain 68 stress points to assess maternal stressors it is presented as either (Not present) it takes score equal Zero or (present) it takes score equal 1.
•The total scoring was calculated and classified as follows:
•The women who had total score of 0-20 were classified as having low coping level.
•The women who had total score of 21-40 were classified as having moderate coping level.
•The women who had total score of 41-68 were classified as having high coping level
2nd part : coping strategy sheet : which contains 30 coping items to assess mother coping strategy during post natal period(immediate post natal period during 24 h ,after 3 days and one week post delivery) for physical , psychological ,and social condition.
The total scoring was calculated and classified as follows:
The sheet was consisted of (30) coping items: it’s presented as either (Not done) it takes score equal Zero or (Done) it takes score equal 1.
•The women who had total score of 0-10 were classified as having low coping level.
•The women who had total score of 11-20 were classified as having moderate coping level.
•the women who had total score of 21-30 were classified as having high coping level
- Results:
The results of this study showed the following findings:
-Almost all women were utilized high level of coping strategies (98.8%), few of them (1.2%) were using moderate level, and. On the other hand, the majority of women (95.3%) were suffering from low and moderate levels of maternal stresses during the puerperium.
-The relation between the coping strategies utilized by the mothers during puerperium and their general characteristics. As regards their ages, the mothers from both age groups had more or less the same pattern in using the 30 elements of coping strategies; the difference is not statistically significant (p = 0.5449). Also all mothers who had master degree (4 mothers) were utilizing coping; also, the majority of women who had middle (Diploma) and university education (99.0% and 99.6% respectively) utilizing coping compared to (92.5%) of read and write mothers, however the differences are not statistically significant (p = 0.6238).
In addition, the majority of mothers (99.1%) who were living in urbanized areas were utilized coping strategies more than the mothers who were living in rural areas (96.6%); yet the difference is not statistically significant (p = 0.1293). Moreover, both housewives and working mothers had the same percentage of coping (98.8%).
- Pray and trying to approach God element comes first in the list of the 30 elements of coping strategies utilized by the women during early puerperium it represents 98.8% of the studied group.
- Crying, shouting & screaming element comes last in the list of coping strategies utilized by the women during early puerperium.
- A positive significant correlation between the maternal stressors and coping strategies was found (r = 0.448).
