Abstract

The CVS is the sudden appearance of neurological symptoms when sever interruption of the blood flow to the brain cell occurs leading to necrosis. The CVS is the third leading cause of death in the United States of America (USA), and the most common cause of long-term disability in adults. Over 150,000 people with CVS die each year. There are approximately 5.5 million CVS survivors in the USA and it is estimated that about 13 million individuals have sustained a so-called “silent” CVS (NINDS, 2010).
Knowledge and use of evidence-based practice are essential to ensure best nursing practice. Nurses must be knowledgeable of this Initiative to support clinical practice toward improved patients’ outcomes. A high quality performance of the nurses would save much of the direct and indirect costs saving (Duncan, 2005).
It is important to assess the nurse’s knowledge and practice to identify their actual performance for the patient with CVS, determine the comprehensive nursing care that necessary for prevention of complication which is increases a vital consideration in order to use as a guide for comprehensive nursing care for patient with CVS to be generalized in ICU (Duncan, 2005)..
This study aims to assess the nurses’ performance for caring of patients with CVS in ICU through the following:
1. Assessment of nurses’ level of knowledge regarding caring for patients with cerebrovascular stroke in the Intensive Care Unit.
2. Assessment of nurses’ level of practice regarding caring for patients with cerebrovascular stroke in the Intensive Care Unit.
Subjects and Methods:
Research design:
A descriptive exploratory design was followed to achieve the aim of this study.
Setting:
The study was conducted in Emergency Medicine ICU, Geriatric ICU and Neurological ICU at Ain Shams University Hospital.
Subjects
A convenience sample of all available nurses (no=40nurses) working in the previous mentioned setting caring for patients with CVS in the pervious mentioned settings at the time of data collection were recruited in this study.
The following tools were used to accomplish the purpose of the study:
I- Self-administered questionnaire Sheet: It was used to assess nurses’ level of knowledge regarding nursing management for patients with CVS in ICU. It was developed by the researcher and written in Arabic language based on review of relevant recent literature and modified by the researcher.
II- Nurses’ practice observational checklist for nurses caring of patients with CVS in ICU: This tool was adopted from Lynn and Lebon, (2008) and Miller and Mink (2009). It was modified by the researcher to assess the actual nurse’s practice in caring for patient with CVS in ICU.
The result of the study revealed the following:
ØRegarding age and gender, the result revealed that slightly less than three quarters (70 %) of the study nurses their age ranged from 20- 30 years and the majority (97.5 %) of them were female.
ØConcerning to their level of education and years of experience; 42.5% of them were diploma and 30% were university degree in nursing. 52.5% had less than 5 years and majority (92.5 %) of them did not receive any training courses regarding caring of patients with CVS in ICU.
ØConcerning the total knowledge, more than two third (67.5%) of the study nurses had unsatisfactory level of total knowledge regarding caring of patients with CVS in ICU.
ØConcerning the items of knowledge regarding caring for patients with CVS, most of the study nurses had unsatisfactory knowledge regarding definition and types, risk factors, clinical presentation, diagnostic investigations and complications (77.50, 72.50, 92.50, 77.50& 87.50) respectively. Also, near two third (65%) of them had unsatisfactory knowledge regarding nursing intervention.
ØRegarding total practice, the study result revealed that all the study nurses (100%) had unsatisfactory practice regarding caring for patients with CVS in ICU
ØConcerning the items of practice, the results revealed that all the study nurses had unsatisfactory practice regarding nursing intervention. Also, about two third (68%) and majority (98%) of them had unsatisfactory practice regarding primary and secondary assessment respectively.
ØThere were no statistical significance correlation between total knowledge and total practice of the study nurses in caring for patients with CVS in ICU (r=+0.237 at P > 0.05).
ØThere were statistically insignificant relation between the nurse’s level of knowledge regarding to caring for patients with CVS in ICU unit and all demographic characteristics except marital status, job place (F=10.907& 3.552 respectively at P<0.05).
ØThere was statistically insignificant relation between the nurses level of practice and their all demographic characteristics of the study nurses (age, gender, marital status, level of education, years of experience, job, job place and training courses) (F=1.109, 0.071, 1.718, 0.962, 1.478, 0.838, 1.176& 0.160 respectively at P>0.05).
