Abstract

Colorectal cancer affects many people in the developed world, and resection is the only curative treatment. Traditionally, cancers of the colon are removed through large abdominal incisions (open colectomy) or by use of laparoscopy.
Laparoscopic surgery has many advantages over open surgery that includes: less surgical trauma, less postoperative pain, earlier resolution of postoperative ileus, quicker resumption of diet, a shorter hospital stay, and an earlier return to social life.
Laparoscopic colectomy carries some disadvantages such as hand tremors, limited degrees on movements of the instruments, Surgeon’s fatigue due to standing for a long operative time, major dependence on an assistance for visualization (camera man) and retraction of the colon or mesentry and significant learning curve for laparoscopic colectomy.
Robotic colectomy overcomed these disadvantages as the surgeon remains seated on the console and does not need an assistant. Also, there are no tremors due to stable well fixed and computerized motions of the instruments which have seven degrees of movements and the shorter learning curve of robotic colectomy.
In robotic colectomy, the surgeon does not depend on an assistant due to the fourth robotic arm that is used for retraction. Moreover, the 3 dimentional image enable the surgeon to demonstrate small structures like blood vessels and ureters to avoid injuring them. Also, there are no possibility for the surgeon to be exposed to infection as he is seated away from the patient.
For the above reasons robotic colectomy has the superiority over laparoscopy in improving oncological outcomes for the patients.
These advantages pushed many centers all over the world to have a robotic device to apply this new technique in practice but there are some limitations that make the usage of robot in surgery somewhat limited. These limitations includes the high cost, the big sized device that needs large operative theatures and the need for upgrading these computerized systems when new version are developed.
So it is concluded that robotic colectomy is an accepted alternative to conventional laparoscopic surgery for colorectal cancer.
