Abstract

Penodental disease has been called the sixth complication of diabetes mellitus. Diabetes, particularly type II diabetes, is a looming health issue with many ramifications because diabetes alters the cellular micro environment in many different types of tissues; it causes many complications. Two cellular process could be affected by diabetes, these are inflammation and a poptosi peuodontal disease is one of the major oral problems encountered in patients with diabetes mellitus. Vascular changes, neutrophil dys function, altered collagen synthesis and genetic predisposition observed in diabetes mellitus may contributed to penodontitis; and the vascular alterations observed is such patients may depend on vascular endothelial growth (VEGF) actions. The presence of cells that are positive for active caspase-3 is considered hallmark of a poptosis activation.
The aim of this study is to evaluated the effect of type 2 diabets and chronic penodontitis on apoptosis (caspase-3 level as a marker), and on vascular endothelial growth factor (VEGF) level in gingival crevicular fluid.
This study involved 15 patients classified as follows: group 1: 15-mild to moderate gingivitis patients. group 2: 15-mild to moderate chronic penodontitis patients. group 3: 15- non-insulin dependent (type II) diabetic patients with mild to moderate chronic penodontitis.
A ssessment of the clinical parameters was carried out at base-line, lmoth and 3 months. Plague index (PI), modified gingival index (MGI), clinical probing depth (PD), clinical attachment level (CAL).
Gingival crevicular fluid sample collection (GCF) using what man 3MM- chromatography filter paper Biochemical study to detection of caspase-3 and vascular endothelial growth factor by enzyme linked immunosorbant asscy (Elisa Kit).
Our results reveal that, in group I there was significant decrease in PI and MGI while, there was insignificant decrease in PD and CAL at base line, 1 month, 3 months.
