Abstract

Endometriosis is present in 20% to 40% of women who complain of subfertility, although it can be found in 5% of fertile women. Laparoscopy is the standard technique for visual inspection of the pelvis and establishment of a definitive diagnosis. Diagnosis of endometriosis can be confirmed only by a surgical procedure (generally laparoscopy) to excise and histologically evaluate disease implants. Treatment for endometriosis includes expectant, medical or surgical treatment. Surgical treatment (either via laparoscopy or laparotomy) includes lesion removal and adhesiolysis, endometrioma resection, LUNA and hysterectomy with or without bilateral oophorectomy. The laparoscopic approach for the management of ovarian endometriomata (OMA) is favoured as it offers many advantages. Currently the commonest procedures for the treatment OMA are either excision of the cyst capsule (laparoscopic ovarian cystectomy) or drainage (fenestration) and bipolar electro-coagulation of the cyst wall. The aim of this work is to throw more light on laparoscopic management of ovarian endometriomas, and to compare laparoscopic excisional surgery versus laparoscopic ablative surgery as regards success (pregnancy) rate and recurrence rate.
