Abstract Anal abscesses and fistulas are a common surgical condition. Management of the majority of anal abscesses and fistulas is straight forward and is based on a sound knowledge of the anatomy of the anorectum. A small percentage of anal abscesses and fistulas are complex and very challenging to manage surgically. The joint aims of maintaining perfect continence and curing the fistula are often difficult to achieve. The treatment of complex anal fistulas is one of the most challenging aspects of colorectal surgery. Aims of treatment are to drain any sepsis, define the anatomy of the fistula and lay open the primary track (fistulotomy). These actions must be performed without impairing continence. |
