Abstract

Jaundice is the most common condition in newborn that requires medical attention in diagnosis and treatment.
The aim of our study is to asses and classifies significant neonatal jaundice and compare between clinical assessment through cephalocaudal progression and laboratory results to detect significant jaundice which requires intervention by phototherapy or exchange transfusion.
This study was done in Minia university hospital and included 50 neonates and we use the ROC curve to compare between clinical assessment and serum bilirubin level and the area under the curve refers to the diagnostic accuracy of the test.
We divided the body into 4 zones according to Kramer’s method of cephalocaudal progression of jaundice: zone A (face, chest, upper abdomen), zone B (lower abdomen and thighs), zone C (arms and legs) zone D (palms and soles).
In this study we found the time of appearance of jaundice ranged from
(1 -5) days with a mean (2.22±0.82) while the duration of jaundice ranged from
(1- 14) days and the mean was (6.9±5.78) days. Also we found that the postnatal age ranged from (1-12 days) and the mean (7.54 ± 7.5) days, the total bilirbin ranged from (4.9 - 25 mg/dl) and the mean was (14.7 ± 8.6 mg/dl).
In this study 44% of neonates delivered vaginally while 56% delivered by CS, and 58% of neonates were have good suckling and start breast feeding while 42% were poor suckling and did not start breast feeding , they were 34% males and 66% females .
The etiology was undetermined in about 50% of cases , 24% sepsis ,18% ABO incompatibility and 8% had cephalhematoma.
